By leveraging the expertise of our global parent company, the world’s leading innovator in kidney care and the largest provider of dialysis products and services, Fresenius Health Partners is able to better manage and coordinate care for individuals with chronic kidney disease (CKD) and end-stage renal disease (ESRD).
A unique care management platform is the cornerstone of all Fresenius Health Partners programs. The Care Navigation Unit (CNU) is a group of highly specialized dialysis nurses and care technicians that provide 24/7 patient support to clinics, hospitals, and nephrology practices, as well as to patients directly.
The CNU ensures patients are getting the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors.
Result: Our patients move from high to lower risk on the healthcare continuum while reducing overall costs.

Sub-Capitation Arrangements under which Fresenius Health Partners uses its unique patient-centric renal health home model to manage the care of CKD & ESRD patients insured by other entities.
ESRD Seamless Care Organizations (ESCOs)
Launched October 1, 2015, Fresenius Seamless Care ESCOs are partnerships in which groups of nephrologists work closely with Fresenius Medical Care to provide highly coordinated care for their Medicare patients. Patients are still on Medicare and their benefits do not change, but their care is monitored and supported by the CNU.
Chronic Special Needs Plans (CSNPs)
Launched January 1, 2016, Fresenius Health Plans’ Medicare Advantage insurance product is designed specifically for ESRD patients. Most Medicare Advantage plans prohibit individuals with ESRD from enrolling, so with the launch of these plans many of our patients are finally able to get the healthcare coverage they need. Fresenius Health Plans replaces a patient’s original Medicare, combining Part A (hospital), Part B (doctors), and Part D (drugs) coverage into one plan. Members also receive bonus benefits not available under Original Medicare, such vision and dental coverage.
